مهاجمة المناعة لخلايا الإنسولين تفتح باب الأمل نحو علاج السكري من النوع الأول
أكد د. محمد يحيى الحربي، استشاري الغدد الصماء وسكري الأطفال، وأستاذ مساعد إكلينيكي بجامعة دار العلوم في حوار خاص مع صحيفة "اليوم"، أن مرض السكري من النوع الأول يُعد من أكثر الأمراض المزمنة تعقيدًا من حيث آلياته وأسباب تطوره. وهو مرض مناعي ذاتي لا ترتبط الإصابة به بشكلٍ وثيقٍ بالوراثة كما هو الحال في النوع الثاني من السكري، وإنما يُعد ناتجًا عن خللٍ جيني يدفع الجسم لإنتاج أجسام مضادة تقوم بمهاجمة خلايا "بيتا" المنتجة للإنسولين في البنكرياس، مما يؤدي إلى تدميرها تدريجيًا وحدوث نقص حاد في الإنسولين.
ويشير د. الحربي إلى أن الوراثة تلعب دورًا محدودًا في النوع الأول، حيث إن نسبة إصابة الأطفال إذا كان الأب مصابًا لا تتعدى 6%، وإذا كانت الأم مصابة تتراوح بين 1.3% إلى 2%. في حين تتراوح هذه النسبة 15% في حالة وجود طفل آخر مصاب في العائلة، وقد تزداد نسبتها من 50% لتصل إلى 100% في بعض حالات التوائم المتطابقة. كما ترتفع النسبة في المتلازمات المناعية الذاتية لعدد من الغدد وغير الغدد، التي تشمل أمراضًا متعددة مثل البهاق، وتساقط الشعر، والتي قد تترافق مع السكري من النوع الأول. وذلك بخلاف النوع الثاني من السكري، حيث إذا كان الأب أو الأم مصابين به، فإن نسبة الإصابة في أبنائهم قد تصل من 60% حتى 80%، وهي نسبة عالية جدًا.

وفيما يتعلق بالجهاز المناعي، يُوضّح د. الحربي أن الأبحاث على مدى 30 عامًا سعت لفهم كيفية تحفيز المناعة الذاتية وتطوير وسائل للتدخل المبكر. وتضمنت بعض المحاولات إعطاء الإنسولين عن طريق الفم أو استغلال أدوية مناعية مستخدمة في علاج السرطان، لكن دون جدوى كبيرة. أما الأبحاث الحديثة، فتركّز على تطوير أدوية تستهدف الأجسام المضادة مباشرةً، وتحيط بخلايا "بيتا" لحمايتها وتأخير تدميرها. وتُظهر بعض هذه الدراسات نتائج واعدة بتأخير تطور المرض لفترات تصل إلى خمس سنوات، وهو إنجاز قد يفتح الباب أمام إيجاد علاج وقائي أو شافٍ مستقبلاً.
ويُعد الاكتشاف المبكر للمرض أمرًا مهمًا للغاية، خاصة في العائلات التي لديها تاريخ مع النوع الأول. فبجانب تقليل احتمالية الإصابة بالحماض الكيتوني، فإن المتابعة الدقيقة للأطفال الآخرين من حيث النمو والنمط الغذائي والنشاط البدني تتيح فرص الكشف المبكر، وتخفيف مضاعفات المرض بشكلٍ كبير. وتُسجل السعودية نسب وفاة تراوح بين 2.9% إلى 3.5% عند التشخيص بالحماض الكيتوني، وهي نسب تستدعي تفعيل برامج رصد وتوعية أكثر فاعلية.
